Cumulus picked by another VDC Program

A multi-state Veteran Directed Care program straddling Montana, Idaho and eastern Washington has chosen Cumulus to streamline processes and optimize outcomes for the veterans being served. Area VI Agency on Aging, aka Western Montana Aging Services, manages successful Veteran Directed Care (VDC) programs for Veterans Affairs (VA) Medical Centers in both Ft. Harrison, MT and Spokane, WA.

Cumulus will be utilized to manage VDC processes, which are initiated with a referral from a VA Medical Center. Cumulus will streamline critical aspects of the VDC service model, including eliminating emails and spreadsheets, in order to facilitate assignments to care coordinators for their resulting home visits, assessments, care plans and budget planning. In addition, Cumulus will assist staff with enrollment and general ongoing veteran support. Monthly, quarterly and annual reporting will be facilitated through Cumulus, as will oversight, monitoring and measures.

The VDC program provides VA-approved veterans with opportunities to self-direct their Long-Term Services and Supports (LTSS) and continue living independently at home. With support from Options Consultants, eligible veterans manage their own flexible budgets, decide what mix of goods and services best meet their needs, and hire and supervise their own workers.

Western Montana Aging Services plans, provides, and contracts for services to older Americans, persons with disabilities and veterans within the planning and service area including the Counties of Lake, Lincoln, Mineral, Sanders, and the Flathead Reservation.
