Healthy Living for ME (HL4ME), Maine’s Community Care Hub, has selected Cumulus as its unified technology platform to support statewide health promotion programming and coordinated social care services across Maine.
HL4ME will implement the Cumulus Community Care Hub Portal to replace its legacy system and establish a modern, scalable infrastructure for group programming, referral management, outcomes tracking, and healthcare integration.
HL4ME serves as Maine’s statewide community-integrated health network, working in partnership with Area Agencies on Aging, health systems, community providers, and other stakeholders to deliver evidence-based health promotion programs and coordinated social care services.

Under the agreement, Cumulus will serve as HL4ME’s central platform for:
- Group programming management, including registration, attendance tracking, survey collection, and automated participant communications
- Unified health promotion and social care coordination workflows
- Public-facing schedules and registration tools to streamline workshop discovery, enrollment, and partner coordination across the state
- Advanced reporting and dashboards to support operational oversight and performance monitoring
- Standards-based interoperability using HL7 and FHIR APIs to enable secure exchange with healthcare partners
- Configurable billing workflows to support Community Health Integration and future reimbursement models
As community care hubs continue to play a critical role in bridging healthcare and social services, HL4ME’s adoption of Cumulus represents another step forward in building resilient, interoperable infrastructure for community-integrated care.
