Community Care Solutions, the community care hub established by the Southern Alabama Regional Council on Aging (SARCOA) to contract with health systems and payers, has selected Cumulus as its technology partner and digital hub.
Beginning with its care coordination program, Community Care Solutions will leverage Cumulus to streamline the import and triage of incoming referrals, care team assignments, care coordination, claims processing, monitoring, and outcome measurement.
