Cumulus Care, Inc. today announced that AgeOptions has selected the Cumulus platform to support Illinois Pathways to Health (ILPTH), a statewide Community Care Hub connecting older adults and individuals with chronic conditions to evidence-based programs and coordinated social care services.
Through this partnership, Cumulus will serve as the central technology platform for AgeOptions’ community-integrated health network, unifying group program delivery, care coordination, reporting, and partner collaboration into a single system.
With Illinois Pathways to Health, AgeOptions is working with its network of community-based organizations to align social care and falls-prevention and health promotion programs with healthcare payers and value-based care models. The Cumulus platform will support both evidence-based workshops and comprehensive care coordination workflows within a unified environment.

The implementation will support a wide range of capabilities, including:
- Statewide management of evidence-based workshops, including registration, attendance tracking, and outcomes measurement
- Public-facing program discovery and enrollment, with integrated surveys and participant engagement tools
- End-to-end care coordination workflows, including intake, assessment, referrals, and follow-up
- Real-time dashboards and reporting to support program oversight, performance monitoring, and funder requirements
- Standards-based interoperability, including HL7 and FHIR APIs, to enable integration with healthcare systems and electronic health records
By consolidating these functions into a single, secure, and HITRUST certified platform, AgeOptions will reduce administrative burden, improve data quality, and strengthen its ability to collaborate with community partners and healthcare organizations across Illinois.
