Illinois Community Care Hub Selects Cumulus

Cumulus Care, Inc. today announced that AgeOptions has selected the Cumulus platform to support Illinois Pathways to Health (ILPTH), a statewide Community Care Hub connecting older adults and individuals with chronic conditions to evidence-based programs and coordinated social care services.

Through this partnership, Cumulus will serve as the central technology platform for AgeOptions’ community-integrated health network, unifying group program delivery, care coordination, reporting, and partner collaboration into a single system.

With Illinois Pathways to Health, AgeOptions is working with its network of community-based organizations to align social care and falls-prevention and health promotion programs with healthcare payers and value-based care models. The Cumulus platform will support both evidence-based workshops and comprehensive care coordination workflows within a unified environment.
