Oregon VDC Program Picks Cumulus

Senior & Disability Services, a division of Lane Council of Governments (LCOG), has selected Cumulus to support the launch and ongoing management of its Veteran Directed Care (VDC) program serving veterans in Lane County, Oregon.

LCOG is establishing a sole-proprietor VDC program in western Oregon, working in coordination with the Department of Veterans Affairs to serve veterans who need assistance with activities of daily living and wish to remain in their homes and communities.

Veteran Directed Care enables veterans to take a consumer-directed approach to their care, allowing them to hire and manage their own workers with support from program counselors. The program is designed to serve veterans with personal care needs, as well as those experiencing isolation or caregiver burden.

Lane Council of Governments will implement Cumulus to support its end-to-end VDC workflows, including processing of referrals from the Department of Veterans Affairs, enrollment, care coordination, assessments, development and tracking of spending plans, ongoing case activities, and program oversight. The platform will also support collaboration with the program’s Financial Management Services partner while providing leadership with dashboards and reporting to monitor program performance and outcomes.

In addition, LCOG will implement Cumulus Enroll to automate and streamline the collection, pre-population, and signature of employer and employee forms, reducing administrative burden and accelerating onboarding for both veterans and caregivers.
