Cumulus to Present at HCBS 2026 in Baltimore

We look forward to connecting with customers, partners, and friends at Booth 206 and are excited to lead an Innovation Track session focused on the evolving role of Community Care Hubs.

On Wednesday, August 26 from 2:00–3:00 p.m., Cumulus and Alabama’s statewide Community Care Hub will present From Referrals to Reimbursement: Operationalizing Whole-Person Health Through Community Care Hubs.

Drawing on the real-world experience of Southern Alabama Regional Council on Aging (SARCOA) and Cumulus, the session will explore how Community Care Hubs can move beyond referral management to become an essential infrastructure for whole-person health. Topics will include healthcare-community partnerships, interoperability, closed-loop coordination, automation, data and outcomes, revenue cycle management and claims, and strategies for building sustainable models that demonstrate value to healthcare partners and payers.

And there’s more happening at Cumulus. We’re expanding the platform to support Older Americans Act programs and OAAPS reporting, bringing the same flexibility, interoperability, and connected infrastructure used for CCHs and other whole-person care initiatives to the Aging Network.
