Vermont Community Care Hub Selects Cumulus for Strategic Technology Modernization

Partnership expands Cumulus’s Community Care Hub model to include comprehensive OAA program management and OAAPS reporting.

Cumulus Care is pleased to announce that the Vermont Aging Network Consortium Inc. (VANC), Vermont’s statewide Community Care Hub, has selected Cumulus following a competitive evaluation process to support a strategic technology modernization initiative for long-term services and supports, federal reporting, and health information exchange.

The initiative marks an important next phase in the evolution of the Cumulus Community Care Hub model. Working with VANC as an anchor collaborator, Cumulus will expand the platform to include comprehensive Older Americans Act program management and Older Americans Act Performance System, or OAAPS, reporting alongside the healthcare contracting, care coordination, interoperability, analytics, and other capabilities already supporting Community Care Hubs.

For VANC, the initiative is part of a broader strategy to build a more integrated and sustainable infrastructure for Vermont’s Aging Network. VANC brings together Vermont’s five Area Agencies on Aging to strengthen coordination, leverage collective capabilities, build sustainable healthcare partnerships, and support the Aging Network’s ability to operate as an integrated delivery system.

By enabling information to move securely across aging services, healthcare, Medicaid, and community-based systems, we can give providers and partners a more complete understanding of each person’s needs, reduce gaps and duplication, and coordinate services around the whole person. For Cumulus, the VANC partnership reflects the next phase in its mission to be the strategic technology choice for modern Community Care Hubs and for unified community care more broadly. Hubs increasingly need to bring multiple programs, payer relationships, grants, initiatives, reporting requirements, and community partners into a common operating environment, rather than managing each opportunity through a separate system.

Cumulus is designed to provide that shared foundation while also connecting the medical and community sides of healthcare. By linking community-based organizations with health plans, providers, health information exchanges, Medicaid systems, and other healthcare partners, Cumulus helps Community Care Hubs operate as an integrated part of the broader healthcare ecosystem.

Supporting OAA and Medicaid Long-Term Services and Supports

A central part of the initiative will be the expansion of Cumulus to support the operational and reporting needs of Area Agencies on Aging.

Cumulus and VANC will work together to develop comprehensive OAA program management and OAAPS reporting capabilities, including workflows, assessments, case management, service tracking, reporting, and related program functions. VANC will serve as a critical subject-matter partner throughout development, contributing the practical expertise of Vermont’s Area Agencies on Aging and providing substantive input on program operations, federal reporting requirements, user workflows, and data needs. This partnership will help ensure that the OAAPS functionality is informed by real-world Aging Network experience and is designed to be accurate, practical, and responsive to the needs of agencies administering OAA programs. The initiative is also intended to improve data quality, standardize workflows, reduce duplicate data entry, and lessen the administrative burden associated with federal reporting.

The platform will also support Vermont’s Choices for Care program, the state’s Medicaid long-term services and supports program for older adults and adults with physical disabilities, including home and community-based services.

Bringing these functions into a common environment will allow participating organizations to manage core Aging Network programs while creating stronger connections across OAA, Medicaid, healthcare, and Community Care Hub activities.

Connecting Vermont’s Aging Network with the Statewide HIE

Interoperability is another major component of the initiative.

Cumulus will integrate VANC with Vermont Information Technology Leaders (VITL), which operates Vermont’s statewide health information exchange. The planned integration is a concrete example of the role Cumulus is designed to play in connecting the medical and community care sides of healthcare, creating a stronger flow of information between Vermont’s clinical infrastructure and the community-based organizations supporting older adults and people with disabilities.

That connectivity is central to the Community Care Hub model. It allows community-based organizations to participate more fully in healthcare delivery, care transitions, care coordination, and value-based relationships while preserving the local relationships and community expertise at the core of their work.

The broader VANC interoperability strategy will also include integration with Gainwell Medicaid, ServTracker, and the BRITE API, creating a more connected technology environment across aging services, Medicaid, healthcare, and community-based care.

Building a Platform for What Comes Next

Cumulus will provide VANC with a common environment for care coordination, case management, program administration, reporting, analytics, interoperability, and collaboration across participating organizations.

The platform is designed not only to support VANC’s current programs and relationships, but also to provide a long-term foundation for additional payer relationships, grants, care coordination arrangements, programs, and Community Care Hub initiatives as priorities evolve.

Implementation is expected to begin in October 2026, with development and deployment continuing through 2027.
