Community Assistance and Transition Care of Houston (CATCH), a community-based organization serving Greater Houston, has selected Cumulus as the technology platform for its expanding Community Care Hub (CCH). Cumulus will support CATCH’s growing portfolio of multi-payer contracts, integration with its clinical electronic health record (EHR) and its participation in the federal Guiding an Improved Dementia Experience (GUIDE) care model.
Cumulus will give CATCH a cloud platform for intake, screening, care navigation, caregiver support, referrals, performance metrics, and contract-specific billing. By unifying these workflows into Cumulus’ modern platform, CATCH will be able to streamline operations, reduce administrative burden, and deliver consistent, high-quality support across all its programs.
